Your SOVA representative can walk you through the following products and forms to help you maximize your One Talk Toolbox options and get your account created in order to establish your service with Lincoln Communications.

Now that you have selected your products, your Verizon or SOVA representative can help you complete the following forms for us to port your existing numbers and get your account creation started.


Local Number Portability (LNP) allows you to move your current telephone number from your current telephone service provider to Lincoln Communications Group. Your current provider requires this Letter of Authorization (LOA) as proof that you have explicitly requested and authorized Lincoln Communications Group to transfer your telephone number(s). By submitting this form, you authorize us to initiate the process of transferring your telephone number(s) to Lincoln Communications Group. After the transfer is completed, your transferred telephone number(s) will be part of your Lincoln Communications Group account.

Recurring Billing Credit Card Authorization (RBCCA)

The security of your payment information is our highest priority. Your card details are encrypted directly in your browser using Authorize.net's Accept.js technology — a Visa company and one of the most trusted payment processors in the United States. Your raw card number is never transmitted to or stored on our servers at any point.

We are fully compliant with Payment Card Industry Data Security Standards (PCI DSS). Your card is stored in Authorize.net's secure encrypted vault solely for the purpose of establishing your recurring monthly billing — eliminating the need for a follow-up call and reducing the exposure of your card details.

By completing this form you authorize Lincoln Communications Group, LLC to charge the one-time setup fees listed below and to retain your payment information securely for recurring monthly billing as agreed.

I, the undersigned authorized account holder, hereby authorize Lincoln Communications Group, LLC ("LCG") to charge the credit card identified above for all one-time setup and activation fees as itemized in the Order Summary below.

I further authorize LCG to retain my payment information, stored exclusively in Authorize.net's PCI DSS-compliant encrypted vault, for the purpose of processing recurring monthly service charges for the products and plans selected above. Monthly charges will begin on my service activation date and will continue until I provide written notice of cancellation.

I acknowledge and agree that: (1) recurring charges will be billed at the rates stated above, with overages billed at the per-unit rates specified for each service; (2) it is my sole responsibility to monitor my credit card statements for accuracy; (3) any disputed charges must be reported to LCG in writing within 30 days of the billing date; (4) in the event a charge is declined, LCG reserves the right to suspend service until payment is received; and (5) to update payment information or cancel services, written notice via email to customerservice@lincolncg.com is required. Cancellation requires a minimum of 60 days written notice per Lincoln Communications Group's Terms and Conditions.

This authorization shall remain in effect until cancelled in writing by either party.
